Medicare Advantage Plans - Illinois

Medicare Advantage Plans are health plan options that are part of the Medicare program. If you
choose to join a Medicare Advantage Plan, you typically get all your Medicare-covered health
care through that plan. This coverage can also include prescription drug coverage.
Medicare Advantage Plans include:
Medicare Health Maintenance Organization (HMO)
Preferred Provider Organizations (PPO)
Private Fee-for-Service Plans
Medicare Special Needs Plans
Medicare Advantage Plan members are issued a health insurance card for your health
care. In most of these plans you will receive extra benefits and lower copayments than in
the Original Medicare Plan. However, you may be required to see doctors that belong to
the plan or go to certain hospitals to get services.
You must have Medicare Part A and Part B to join a Medicare Advantage Plan. You will
have to pay your monthly Medicare Part B premium to Medicare. In addition, you might
have to pay a monthly premium to your Medicare Advantage Plan for the extra benefits that
they offer. If you have a Medigap policy, you may want to discontinue it if you join a
Medicare Advantage Plan. It is not required that you do so, but your Medigap policy will not
pay any deductibles, co-payments, or other cost-sharing under your Medicare health plan if
you are enrolled in a Medicare Advantage Plan.

Medigap (Illinois Medicare Supplemental Insurance)

Medicare supplemental insurance policies that are sold by private insurance
companies to Medicare beneficiaries to fill the "gaps" in Original Medicare
Plan coverage. Medigap policies help pay some of the health care costs that
the original Medicare Plan does not cover. There are several standardized
policies, Plan A through Plan N. The state of Maine decides which of the plan
policies can be sold in the state. Medigap policies only work with the original
Medicare Plan. If you are in the original Medicare Plan and have a Medigap
policy, then Medicare and your Medigap policy will each pay their share of
covered health care costs.
Plan A Basic Benefits (these benefits also included in Plans B - N)
Medicare Part A co-insurance for the 61st through the 90th day in the
hospital ($283 a day in 2011)
Medicare Part A co-insurance for any hospital confinement beyond the
90th day, up to an additional 60 days during the policyholder's lifetime
($566 a day in 2011)
After using all the Medicare hospital inpatient coverage, including the
lifetime reserve days, all Medicare-eligible hospital charges will be covered
up to 365 additional days during the policyholder's lifetime
The reasonable cost of the first three pints of blood each year under
Medicare Part A and Part B
The 20% co-insurance amount under Medicare Part B (doctor and other
medical bills) after the policyholder pays the Medicare Part B deductible
which is $162 in 2011. For outpatient mental health services, the 45% co-
insurance amount after the deductible is paid
The following list describes the additional benefits that may be included in plans B through N.
Medicare Part A Deductible
Payment for all of the Medicare Part A inpatient hospital deductible. The Medicare Part A deductible
is $1,132 in 2011.

Skilled Nursing Facility Care
Coverage up to the co-insurance amount from the 21st day through the 100th day in a Medicare
benefit period for post-hospital skilled nursing facility care ($141.50 a day in 2011)
Medicare Part B Deductible
Coverage for all of the Medicare Part B deductible ($162 in 2011). Medicare Part B covers doctor
and other medical bills.

100% of the Medicare Part B Excess Charges
Coverage for all of the difference between the actual Medicare Part B charge as billed and the
Medicare-approved Part B charge not to exceed any charge limit set by the Medicare program or
state law.

Medically Necessary Emergency Care in A Foreign Country
Coverage for 80% of the billed charges for Medicare-eligible expenses for medically necessary
emergency care received in a foreign country. Care must have started during the first 60 days in a
row of each trip outside the United States. This benefit has a $250 calendar year deductible and a
lifetime maximum benefit paid of $50,000.
High Deductible Plan F
Plan F has an option called High Deductible Plan F offered by several companies. This high
deductible plan offers the same benefits as the regular Plan F but the benefits do not start until
after you pay a calendar year deductible of $2,000 (this deductible can go up every year). Some
of the expenses you will have to pay to satisfy this high deductible include the Medicare
deductibles for Parts A and B, but does not include the Plan F Foreign Travel Emergency
deductible. The Foreign Travel Emergency deductible must be paid regardless of whether you
have met the $2,000 high deductible. Also, you can't count the Foreign Travel Emergency
deductible toward the $2,000 high deductible
Medicare SELECT - Illinois
Medicare Select policies are also standardized plans, however, to receive full hospital benefits
you must use participating hospitals that are in the network. These insurance policies usually
cost less because there are restrictions that affect where you can go to get treatment. If you do
not follow the Medicare Select policy requirements, Medicare will still pay its portion of the bill,
however, the Medicare Select company is not required to pay the full benefits under the policy
and will deny payment or pay less than the full benefit if you go outside the network for non-
emergency services. Consult with your Medicare insurance agent for updated availability
information.
